NEW HIRE FORMS MAY NOW BE COMPLETED ON LINE

We are excited to announce that your new employee paperwork may  now be completed and submitted electronically. To access the online version,  Click Here to access. Please make sure to download the completed application to keep for your records.

Once you have completed and submitted all the forms, our New Hire Team will review all the information you provide, and determine if the employee is eligible for hire. You will be able to attach copies of driver’s licenses, social security cards and any other documents needed.  At this time, you and your prospective employee will need to complete the form together.

Remember:  You cannot offer a position or allow a prospective employee to start work until they have been cleared by the CDS office. If additional information is needed from you, a member of our New Hire Team will contact you with further instructions.  IMPORTANT: Any hours worked prior to approval by the CDS office will not be paid.
